Senior Care Services is a Medical Practice specializing in remote Tele-Health services for medical practices around the country. We perform medical records updating, medications reconciliations, pre-AWV documentation, and patient outreach in between regular office visits with your primary care physician at the clinic. Senior Care Services bills Medicare for our services.
Our Senior Care program is designed to reach out to patients to identify care gaps, new diagnoses, opportunities to improve care, and review existing conditions which may be present in the EMR or have fallen off the patients records over the years. We encourage participation in the program and visits to the practice, and prep the patient for their AWVs, and assist with scheduling patients into the practice to complete their AWVs. We also help identify patients who might be eligible for remote patient monitoring and assist the patient with set up for RPM with the practice. We also perform triage services for patients who have medical concerns and have not yet spoken to the practice, or seen recently by their PCP.
We send out letters and emails to patients explaining the program and invite patients to participate. We then call the patients to conduct Tele-Health visits with our Nurse Practitioners. We then send our notes to the primary care physician and the clinic for review and the doctor decides what information compiled by the Senior Care Services nurse gets added to the patients medical records.
